How Can We Be More Like Vietnam?: The Critical COVID-19 Question We Should Be Asking When it comes to controlling the COVID-19 pandemic, nobody did better than Vietnam. What did they do and how can we do the same thing?
Research News
SUMMARY
- Studies of mask use in China** and of preventive behavior in Vietnam** during the pandemic found that 99.7% and 99.5% of subjects wore masks. This near universal use of masks sharply contrasts with the limited use of masks in the United States and may be an important part of the explanation for why those countries managed the COVID-19 pandemic so much better than the US.
- An exhaustive meta-analysis of clinical trials** and a registry study both found strong evidence that the use of hydroxychloroquine confers no beneficial effects and is associated with an increase in adverse reactions in COVID-19 patients. Taken together, these studies suggest that the continued use of hydroxychloroquine in this context should be considered medical malpractice.
- Expression of the gene for the ACE2 receptor, the target for the SARS-CoV-2 spike protein, increases with age and is dramatically lower in young children suggesting a potential explanation for the lower rate of severe disease in younger age groups.
- An analysis of human breast milk from two mothers with COVID-19 found SARS-CoV-2 in repeated samples from one mother.
- A study of a Seattle nursing home found that a coordinated public health intervention combining isolation, protective measures, and testing effectively contained the outbreak. This model should be studied as a standard for proactive prevention of COVID-19 at nursing homes and assisted living communities nationwide.
- Autopsies of 8 COVID-19 victims found SARS-CoV-2 (by immunohistochemistry) in the lungs and airways, but nowhere else, despite evidence of effects related to multiple organ systems. Given the growing belief that this is a multi-system disease, these findings have major implications, particularly if borne out by additional studies.
- The shift in the growth rate epidemic curves** of countries around the world following the introduction of public health interventions suggests that more than 60 million confirmed cases of COVID-19 were averted by these measures, which, given the current global rate of fatalities per confirmed case of 6.8%, translates to more than 4 million deaths averted worldwide.
- An analysis of 15,000 SARS-CoV-2 genomes** found none that were convincingly associated with increased viral transmission. It appeared that recurrent mutations are either neutral or weakly deleterious.
- Sequencing B cell receptor repertoires** from COVID-19 patients has generated a library of 777 antibody clones in reaction to the virus which should provide a foundation for further studies related to vaccines and immunotherapy.
