SARS-CoV-2 detected in Italian wastewater last December Analysis of samples collected and stored as part of an ongoing study of wastewater in Italy found viral RNA in Milan and Turin on December 18. The limits of sensitivity for wastewater testing suggest that infections had been actively spreading since early December, at least 2 months before the first case of COVID 19 was diagnosed.
Seroprevalence studies in six states find SARS-CoV-2 infection rates 6-24 times reported number of cases Analyses of 11,933 residual clinical specimens between late March in April found antibodies in 1.1% (WA) to 6.9% (NYC) of tests. The use of residual clinical specimens eliminates the potential for self-selection bias present in studies using volunteers (see blog post below). On the other hand, it is conceivable that some of the samples came from patients being seen for conditions not thought to be related to COVID-19 that are actually due to SARS-CoV-2, making these overestimates of seroprevalence. Other studies of blood bank samples show good concurrence with residual clinical samples, making that error less likely. The ratios of estimated infections (based on seroprevalence) to reported cases were 11.2 (Puget Sound), 11.9 (New York City), 11.2 (South Florida), 23.8 (Missouri), 10.5 (Utah), and 6.0 (Connecticut). These results indicate that the actual number of infections is roughly 10 times the number of reported cases, suggesting the disease is less deadly, but far more contagious than previously recognized. If these numbers are correct, the true numbers of cases in the US would be close to 25,000,000.
Large autopsy study of COVID-19 patients concludes infection is almost exclusively limited to the lungs. Detailed autopsy studies of 41 consecutive COVID-19 fatalities found "massive presence" of viral RNA in the lungs with extensive damage to the alveoli along with widespread thrombi (blood clots) in the small and large vessels of the lungs. There were limited signs of infection in the heart and no evidence of the viral infection in kidney, liver, or brain. Damage to those organs appears to be the combined result of the age of the patients and hypoxia related to COVID-19 lung injury. It should be noted that not all organ systems were examined in all patients, but the reasons for this were not discussed. This raises the possibility of bias in the study results. In addition, these results appear to be at odds with the two studies below describing injury to the pancreas and brain.
New onset diabetes is frequently associated with critically ill COVID-19 patients A retrospective study of 120 patients in China found that 54% of critically ill patients with no history of diabetes and no treatment with steroids had fasting blood glucose levels consistent with diabetes, almost four times the rate seen in moderately to severely ill patients. The authors conclude this is an indicator of multi-organ injury, including infection of pancreatic islet cells, and should be used as an indicator of risk in hospitalized patients.
SARS-CoV-2 replicates in neurons and in the brains of mice A team of investigators at Yale demonstrated that the virus can infect neuronal cells in culture, replicate in those cells and cause the death of surrounding neurons. When the virus was injected into the cerebral ventricles of mice at just 1% of the concentration necessary to infect the lungs with intranasal injection, the virus caused severe brain injury and death. This suggests that SARS-CoV-2 infection of the brain has the potential to cause serious brain injury and even death.
